Home Based Primary Care
Home Based Primary Care Program
Home Based Primary Care (HBPC) is the name for primary care provided to Veterans in their homes. Services are provided by a team supervised by a VA physician. The team includes a provider (physician, nurse practitioner or physician assistant), nurses, rehabilitation staff, a social worker, and a dietitian. This program may be right for Veterans with complex health care needs who have a hard time getting to clinic. HBPC may also be right for patients who are isolated or whose caregiver needs support and assistance. Home Based Primary Care can be used in combination with other Home and Community Based Services.
Home Based Primary Care is part of the VHA Standard Medical Benefits Package. So all enrolled Veterans are eligible IF they meet the clinical need for the service and it is available. A copay for Home Based Primary Care may be charged based on your VA service-connected disability status and financial information. You may have a basic copay each time a VA staff team member comes to your home for a medical visit (the same as if you went to a VA clinic).
If you qualify for Home Based Primary Care, your care plan will include:
• Primary care visits at your home by a physician, nurse practitioner or physician's assistant
• Regular nurse visits to your home
• Case management through your nurse
• Help with coordination of your services by your HBPC team
• Evaluation by a physical or occupational therapist, with ongoing therapy visits in the home if needed
• Nutrition evaluation and counseling from a dietitian
• Help with managing your medicines
